Hospitals around the country are now banning vaginal birth after cesarean delivery

Hospitals around the country are now banning vaginal birth after cesarean delivery and insisting women undergo repeat cesarean delivery. Hospitals say they cannot comply with the guidelines issued in 1999 by the American College of Obstetricians and Gynecologists. The guidelines call for a doctor to be available "immediately" throughout active labor when a woman is attempting a vaginal birth after cesarean delivery, in order to perform an emergency cesarean section if needed. In order for a hospital to comply with the new recommended guidelines, a whole operating crew, an anesthesiologist and obstetrician would have to be in the hospital around the clock. Only major medical centers, with in house physicians, will be able to continue to offer vaginal birth after cesarean delivery. Smaller hospitals may now have to abandon the practice.

Why the controversy? Women attempting vaginal birth after cesarean delivery are at a greater risk for uterine rupture than what was once believed. Uterine rupture can be life threatening for both the mother and her baby. Some patients with uterine rupture may require a hysterectomy and some infants may die. The controversy that arises with the risks associated with vaginal birth after cesarean delivery is that many women are willing to take the risk, but their freedom of choice over who controls childbirth is being steamrolled by physicians and hospitals. Women are not being allowed to have a choice in their mode of delivery. Doctors say their position is based on concern for the patient's safety.

Doctors now have a new worry that women trying to avoid repeat cesarean delivery may give birth at home or in birthing centers that are not equipped to perform emergency cesarean surgeries if necessary. Also of concern is the possibility that laboring patients may wait to the last moment to go to the hospital to avoid repeat cesarean section.

The controversy surrounding vaginal birth after cesarean delivery is not only a health care issue, it is becoming a women's civil rights issue. A woman should have the right to choose her mode of delivery. 1) Don't use pregnancy as an excuse to binge.
Pregnancy is the time to get strict about nutrition. Eating junk food will produce a small baby and a big Momma. Make your pregnancy a time to turn over a new leaf if you are a junk food junkie. You'll be developing habits that will serve you well as you teach your children about nutrition.

At the same time, don't obsess about weight gain. I gained between 35-50 pounds for each of my pregnancies, and still didn't have trouble getting back into my old clothes within a few months postpartum. Focus on good nutrition, stay as active as you can, and you won't go wrong.

2) Breastfeed your baby.
Breastfeeding burns around 500-700 calories a day. Wow- that's not bad for sitting around relaxing on the couch! Your body puts on 9 pounds during your pregnancy specifically for the purpose of lactation. The extra weight is laid down on your thighs to make sure you will have enough fat to burn to make milk for baby. Call it "famine insurance" if you will. If you don't breastfeed, guess what? That 9 pounds stays.

Several studies show that nursing Moms return to their prepregnancy weight more quickly. Nursing also helps your uterus contract back to its prepregnancy size faster. Plus you'll have a nicer decolletage to boot!

3) Talk a walk every day
Walking is probably the perfect exercise for new Moms. It's not stressful on your joints (which are still loosey-goosey from the pregnancy hormones for a while after you give birth). It's free and doesn't require a babysitter or any special equipment. It gets you out in the sun, which helps regulates your sleep/wake cycle, causing you and baby to sleep better at night and possibly helping prevent postpartum depression. And you can do it with a friend. If you're sleep deprived and can't bear the thoughts of exercise, call up a buddy and make a date with her to walk several mornings a week. Then you can treat yourselves to Starbucks and gossip afterwards.

4) "Wear" your baby
In addition to the many other benefits, using a baby sling or other soft cloth carrier and wearing your baby will burn lots of calories during the day as you do your household chores or care for other children. It also makes it easier for you to be active. You'll be less tempted to sit around. Throwing on a sling and sitting baby inside it is much easier than lugging out and setting up a heavy stroller.

5) Curb those cravings
Postpartum Moms have cravings for several reasons. One is because serotonin levels are dropping, leading to cravings for sugar. Substitute snacking with another, health producing activity, like getting together with your buddy and going for a walk.

Another cause of cravings may be low blood sugar due to inadequate nutrition (eating too infrequently or eating too much refined sugar). Keep your blood sugar in control by eating frequent small meals that are balanced in nutrients. Keep snacks handy that you can grab quickly before you feed the baby. You need fats, protein and complex carbohydrates. Things like dried fruit and nuts, granola, full fat yogurt (buy the plain kind and mix in all-fruit jelly. Most yogurt has way too much sugar otherwise), kefir, vegetables sliced ahead of time and served with dips, hummus, whole grain pita bread, fruit smoothies, etc will keep your blood sugar constant and provide good nutrition. And keep taking your prenatal multivitamin to help prevent deficiencies that may cause cravings.

6) Avoid excessive caffeine
Drinking too much caffeine leads to insulin surges which cause your blood sugar to drop. When this happens most people head straight for the refined carbs to quicky bring their blood sugar back up. That's no good.

7) Get enough rest
Easier said than done for a new Mom, to be sure! But if at all possible, nap with your baby. When you are terribly sleep deprived, you tend to gain weight. Consider bringing baby to bed with you if you are nursing. You will sleep better, baby will sleep better, and Daddy will sleep better (making it more likely that he will take baby for a walk and let you nap some afternoons!). Women all over the world have been doing it for thousands of years. As long as you are not morbidly obese, using sleeping pills or inebriated, cosleeping is safe.

8) Focus on good nutrition
This is also a challenge for a new, stressed out Mom. Check out books like "The One Armed Cook" for ideas on how to prepare healthy meals with a baby in arms. Visit a La Leche League meeting and ask other Moms how they manage to eat well with kids in the kitchen. Some Moms who were former junk food junkies have done well with the following technique. Before feeding themselves a particular food, they ask: "Would I feed this to my baby?" If the answer is no, they don't eat it.

9) And lastly, give it time
Be realistic- don't expect yourself to be back into your favorite jeans within a few weeks of delivery. In the meantime, appreciate your new, womanly curves (especially the cleavage that breastfeeding gives you!). I noticed that my clothing style changes somewhat after I have a baby. My normal tailored, classic look gives way to a soft, feminine style that flatters my postpartum body more.

Common Pregnancy Problems

All pregnant women will agree that the first few months of pregnancy are unpleasant, to say the least. Morning Sickness

Sipping on fizzy water with lemon in it, or non-caffeinated teas like peppermint and ginger can help calm nausea.

Abdominal Pain or Cramps
There are many changes going on in your body. A lot of stress is being put on your abdominal muscles, which can cause sharp pains and cramps. To relieve these pains, try using a warm heating pad. To prevent these pains, try strengthening your abdominal muscles. Talk to your doctor to find out which kinds of exercises are best for you.

Swelling

Take several naps during the day. Eat a well balanced diet, including about an extra 300 calories per day. Iron deficiency is a leading cause of fatigue, so be sure to get enough. Although it may seem counter-productive, fatigue can often be relieved by exercising. Exercise will be refreshing and will leave you feeling energized.

Heartburn
Many of the remedies associated with nausea will also help you deal with heartburn. In addition, chew fennel or papaya enzymes, and avoid wearing tight clothing.

Bladder Problems
Bladder problems can be among the most bothersome of pregnancy related issues. First and foremost, drink plenty of water. Avoid junk food, refined starches, coffee, sugar, tea, and foods high in acidic content. Avoid using soap on your genitals. Be sure to empty your bladder and wash with water after intercourse. Wipe from front to back after using the bathroom. Try to include cabbage, leeks, and garlic in your diet, drink 3 glasses of cranberry juice every day, and take a vitamin C supplement.

Some women get more problems than others, but they generally get a little easier during the second trimester. In general, just make sure to get plenty of rest, drink plenty of water, get some exercise regularly, eat a well balanced diet, and try to keep an optimistic mindset.

Getting Pregnant

As the weeks turn into months you may begin to realize the cold truth that getting pregnant isn't as easy as you had imagined. If it's your first time you may even have spent most of your life trying to 'avoid' getting pregnant and even fearing it.. well now the time has come you may be starting to realise that it isn't as straight forward as you thought.
There are several factors which may be impeding your getting pregnant.

Why is it so difficult?

Well, logistically, the timing is very important.
A healthy woman ovulates only once every cycle normally releasing an egg which needs to be fertilised within a period of 6 to 12 hours in order to become pregnant.
This is the main factor which stops the majority of healthy couples from becoming pregnant.

The male sperm can survive inside the woman for between 2 and 3 days and the ideal situation for fertilisation is to have the sperm recently deposited inside the woman and waiting for the egg to appear - most fertilisation actually takes place at the mouth of the fallopian tubes.

The quality of the male sperm can vary greatly. Heat around the testicles, the wearing of tight underwear and excessive ejaculation are all factors which can greatly decrease the amount of active fertile sperm reducing the possibility of fertilisation.

So if a couple had intercourse twice a day every day for the whole month it would not necessarily be as effective as to have intercourse only once at the right time... just hours before ovulation occurs.

An effective method used by many couples (which will be addressed in a future article) is the charting method or BBT (Basal Body Temperature) method. This basically involves charting the daily vaginal temperature. Just at the point of ovulation there is a significant rise in temperature. This, combined with other symptoms such as density of vaginal mucus and possible abdominal sensations will show the when ovulation is occurring. After a few months of using this highly accurate method, the couple can predict with high certainty exactly when ovulation will occur and can have intercourse just hours before this is due to occur to ensure fertilisation.

Major health matters that impede fertilization include smoking and drinking in both the woman and the man. Un-healthy eating.. the woman should make sure that she has a good supply of vitamin B12 (found in meat, fish, eggs and milk) also the taking of folic acid can greatly help matters.
Drug taking (including some prescription drugs) is also very harmful for the conception process (and for the unborn baby if continued into pregnancy). The smoking of cannabis / marijuana reduces the amount of active sperm produced by the man by well over 50% and the effects of this last for over 6 months.

All these points show how difficult it can be to get pregnant and you might begin to see that the creation of another human life really is a miracle when you see how improbable the chances of fertilisation really are.

How Much Weight Should You Gain During Pregnancy?

If you're pregnant, you're very likely concerned about the amount of weight you're gaining, the effect it has on your body, even how difficult it will be to take off after your pregnancy. Your OB/GYN or midwife is your best source of advice about healthy weight gain during pregnancy, but there are general guidelines.

Depending on your weight at the start of your pregnancy, your doctor may tell you that a healthy weight gain for you is anywhere between 15 and 40 pounds. If you're underweight to start (a BMI of less than 18.5), 25 to 40 pounds is a reasonable weight gain during pregnancy. If you're overweight, he or she may suggest you stick closer to 15 to 25 pounds. Of that weight, 6 to 8 pounds of it is the baby. The rest is amniotic fluid, extra tissue and blood to nourish the baby (including the placenta), and the increased size of your breasts and placenta. You'll lose as much as 15 pounds of it WITH the birth (amniotic fluid, placenta and baby).

Any doctor will tell you that pregnancy is NOT the time to go on a diet. Your body AND your baby need the nutrients of an adequate, balanced diet to keep you both healthy. This doesn't mean that you should throw all your restraint to the winds and 'eat for two', though. Your body needs approximately an extra 300 calories a day to build a healthy baby. Those 300 calories should come from the same healthy variety of foods that your normal diet gives you. (You were eating a healthy, balanced diet, weren't you? If not, pregnancy is a great time to start.)

You can expect to gain weight along a fairly predictable pattern. In the first three months, you'll gain 2-4 pounds altogether. During the second trimester, you can expect to gain between 3-4 pounds a month (about a pound per week). During the last three months, you'll gain an additional 8-10 pounds. Your doctor or midwife will weigh you regularly, and may express concern over a deviation from this pattern. A sudden sharp weight gain, for instance, can indicate pre-eclampsia or gestational diabetes.

If your doctor advises you to try to limit your weight gain during your pregnancy, be sure to choose a healthy diet that provides all the necessary daily requirements for vitamins, minerals and other nutrients. Remember that a 'diet' during pregnancy isn't meant to help you LOSE weight, but rather to limit the amount of weight gained.

Regular exercise is also good for both you and your baby. You can maintain most of your daily activities, and if regular workouts were a part of your daily routine, then by all means keep them up. You'll feel better, and your body will be less likely to protest the extra weight with aches and pains. Do keep in mind that exercise during pregnancy shouldn't be overly strenuous, and that you should avoid activities with a risk of falling or injury. Also remember that your center of balance is different - it may feel awkward to do the things you usually do while you're carrying your little bundle of joy.
